Realise services brings together the learning disability experience and dedication of two not-for-profit, support and care companies, Isle Care and Somerset Care at Home.
Working across South and South West England, Realise supports over 400 young people and adults with learning disabilities to live full and positive lives.
Who we are
Realise is a community service. Our customers play active roles in their communities, and have a strong voice in the way we deliver services.
Realise supports people across the spectrum of learning disability need. We work with our customers to build quality learning disability services, promote independence, and lobby for resources and change.
Realise recognises people’s abilities, and supports people to pursue their dreams.
Realise also offers advice and information on employment and training, life skills, and personal budgets.
Realise teams provide tailored support packages for people living in their own homes, and for people making the move to independent living.
Realise supports people living in shared housing, accommodation based Supporting People services, and Registered Residential care.
Where we work
Realise offers services across the South and South West of England, working in Surrey, Hampshire, Dorset, the Isle of Wight, Wiltshire, Somerset, Devon, & Gloucestershire.
